George H. Butler was a Gilded Age writer, diplomat, and soldier, who was also a poster boy for what Richard Stott identifies as the “jolly fellow.” Butler parlayed a witty writing style to enter journalism, and with the help of his influencial uncle, Benjamin Butler, he obtained a diplomatic posting as consul general to Egypt. Overly fond of alcohol, George Butler participated in several newsworthy fights, often involving deadly force. His life serves as an archetype for out-of-control manliness, which was contrary to gender expectations of polite society in 1870s America.
Recovery-focused cognitive–behavioural therapy for recent-onset bipolar disorder: Randomised controlled pilot trial

Background
Despite evidence for the effectiveness of structured psychological therapies for bipolar disorder no psychological interventions have been specifically designed to enhance personal recovery for individuals with recent-onset bipolar disorder.
AimsA pilot study to assess the feasibility and effectiveness of a new intervention, recovery-focused cognitive–behavioural therapy (CBT), designed in collaboration with individuals with recent-onset bipolar disorder intended to improve clinical and personal recovery outcomes.
MethodA single, blind randomised controlled trial compared treatment as usual (TAU) with recovery-focused CBT plus TAU (n = 67).
ResultsRecruitment and follow-up rates within 10% of pre-planned targets to 12-month follow-up were achieved. An average of 14.15 h (s.d. = 4.21) of recovery-focused CBT were attended out of a potential maximum of 18 h. Compared with TAU, recovery-focused CBT significantly improved personal recovery up to 12-month follow-up (Bipolar Recovery Questionnaire mean score 310.87, 95% CI 75.00–546.74 (s.e. = 120.34), P = 0.010, d=0.62) and increased time to any mood relapse during up to 15 months follow-up (χ2 = 7.64, P<0.006, estimated hazard ratio (HR) = 0.38, 95% CI 0.18–0.78). Groups did not differ with respect to medication adherence.
ConclusionsRecovery-focused CBT seems promising with respect to feasibility and potential clinical effectiveness. Clinical- and cost-effectiveness now need to be reliably estimated in a definitive trial.

Summary
Nineteenth-century imperialism presented challenges and opportunities to the non-western world. To most states it was a threat, to several a role model. Embracing the “western way of war” required massive investments, plus dramatic political, educational, and social changes. Most attempts to do so were at best partially successful, many a waste of resources.
A few states, like Ethiopia in the 1880s and 1890s, created new military systems that allowed powerful resistance to western imperialism. Elsewhere reformed armies enhanced the coercive powers of the state, reducing opposition to further change. Bandit leaders in the Balkans and Manchuria found their lairs less secure, Taiping rebels were mowed down by Ch’ing (Qing) soldiers who were armed with Enfield rifles, while in Morocco a few Krupp guns guaranteed that the sultan’s annual mahalla would collect tribute quickly.
China’s tzu ch’iang yun-tung (ziqiang yundong), or Self-Strengthening Movement, is a good example of this proposition. A reaction to the the twin disasters of the First OpiumWar (1839–42) and the Taiping Rebellion (1850–64), this movement was the first sustained effort to improve the Ch’ingmilitary. Self-strengthening represented more than the purchase of British rifles or hiring a few military advisors; it called for China to improve revenue collection, construct arsenals and shipyards, and to train more efficient soldiers and sailors.

Analysis of prescribing practices in an acute psychiatric ward

Objectives: Prescription writing is a basic clinical skill for all doctors; however, errors in prescriptions are frequent and can result in significant harm to patients both from sub-therapeutic dosages and from over prescribing. This study examines the prescribing practices of ‘as required’ medications in an acute psychiatric inpatient unit attached to a university teaching hospital.
Method: A clinical audit was conducted to examine both prescribing practices of ‘as required’ medication and recording of ‘as required’ medication in nursing notes. A series of educational interventions were undertaken in an attempt to improve prescribing practices in relation to ‘as required’ medication and two further audits were completed to assess any change in practice and whether such changes were sustained.
Results: Psycho-education led to an improvement in a number of prescribing practices, including increased generic prescribing, improved documentation of indications for medication administration and increased writing of medication dosages in acceptable abbreviations. These benefits were maintained four months after the education interventions. Rates of documenting the frequency of medication correctly and recording the administration of medication in nursing notes remained low.
Conclusion: Our audit cycle clearly demonstrates that education can ameliorate prescribing practices and these improvements were maintained four months after education was delivered. However some aspects of prescribing practice remained deficient, and more focused educational interventions are required in these areas.
